Name brand prescriptions as well as generic prescriptions. Low prices are available at participating retail pharmacies nationwide, as well as through the Home Delivery Pharmacy Service, which delivers medications right to your door.

Dental: Savings average 10% to 50%
Office Visits, Cleanings, X-Ray's, Extractions, Crowns, Root Canal's, Orthodontics, and more.  As a Dental Care Plan member, you and every enrolled member of your family can visit any participating dentist as often as you need and enjoy savings an average of 10% to 50% each visit.

Vision: Savings average 10% to 50%
Designer frames, Lenses (Single Vision, Bifocal, and Trifocal), Contacts, in addition to Ophthalmologist and Optometrist visits.  Our Vision Care Plan offers discounts savings an average of 10% to 50% on a wide variety of vision services and products through over 18,000 participating optometrists, ophthalmologists, and opticians in private practices, as well as leading retail stores.

Alternative Health: Savings average 10% to 30%
Chiropractic, Acupuncture, Registered Dietitian, Herbal Consultant, Holistic Care, Homeopathic Care, Massage, and Meditation.  We have contracted with one of the largest integrated networks of chiropractors and other top qualified complementary alternative medicine practitioners in the country.  As a PlanB for Health member you can expect to receive an average of 10% to 30% savings.

Hearing: Savings average 10% to 50%
Over 200 models of hearing aids are available to you and include many styles.  Our Hearing Care Plan makes hearing services affordable with discounts an average of 10% to 50% on services and products at over 1,600 locations throughout the U.S from one of the largest provider of hearing healthcare services in the country.

Long Term Care: Savings average 10% to 30%
Home Care, Assisted Living, Nursing Homes, Skilled Care, Intermediate Care, and Custodial.  We have designed a plan to assist seniors to find the resources, answers and support necessary to meet and plan for the challenges of aging.

Provides members with a confidential source to discuss problems or personal situations they may be experiencing at home and at work. Accessible by a toll-free number seven days a week, 24 hours a day, callers speak immediately with counselors who provide immediate services.

Patient Advocacy
Our advocacy team works toward helping members who cannot afford their larger medical bills by searching government programs, entitlement programs, grants, and foundations, among other services to aid in the payment of medical bills in addition to negotiating greater discounts, payment plans, and securing discounts in advance of services. This service is provided on a case-by-case situation based on best efforts (no guarantees are implied).

24-Hour Nurse Hotline
 Receive informed decisions about your health and medical care 24-hours a day, 365 days a year for many areas:

• Symptom assessment
• Information about self care techniques for common symptoms
• Information on diagnostic and surgical procedures
• Prescription and over-the-counter medication information

Physicians Referral Program
One of the most frequently reported concerns with many other healthcare savings programs is that their members have difficulty receiving their scheduled savings from physicians and hospitals. To help alleviate this PlanB for Health provides customer service agents for members to call before scheduling appointments for some of the more costly services such as hospital, physician and chiropractic care. Our agents verify that the healthcare practitioner is still in the network and will honor the PlanB for Health program savings. This makes it easier for PlanB for Health members to receive maximum savings without confusion.

Travel Assistance
As a PlanB for Health member you will have access to a worldwide network of medical personnel whenever you travel. Travel Assistance and Emergency Medical Evacuation Services are available when you are traveling more than 100 miles from home and are faced with a serious injury or illness or a travel related emergency.  This benefit includes hospital transportation, repatriation, family visitation assistance and much more.  (All travel assistance services must be pre-arranged by our Program Medical Advisor).

Accident Medical Expense Benefit and AD&D Benefit If you don't have insurance or you are not receiving benefits by another plan this is a must. Accidents happen every minute. One out of four Americans suffers from some type of bodily injury every year that requires medical attention. In the event of an accident, you and your family can have peace of mind that you have protection. PlanB for Health offers four levels of Accident Medical Expense Benefit and Accidental Dismemberment & Death (AD&D) for your entire family. Ranging from a $2500 accident benefit* with $2500 AD&D to a $7500 accident benefit* with $50,000 AD&D, depending on the PlanB for Health program selected. *($100 deductible)

Accident Disability Benefit
If you, the member, become totally disabled, unable to perform the substantial and material duties of your regular job and are under the care of a doctor, after a 14 day elimination period you will receive a Maximum Benefit Amount of $200 week for 26 weeks.

Vision Plus
In addition to the deeply discounted and savings you receive with your Eye Med program with Vision Plus Benefit, you and each of your family members will get one free eye exam a year. The cost of an eye exam can be $72 or more. With Vision Plus available on Signature Care Programs a family of four could save $288 a year with this benefit only.

*Members of PlanB for Health are automatically enrolled in the Consumer Assistance Services Association (CASA).  As a member you have access to the limited insurance policies listed above provided by the association (CASA).  These programs are not Major Medical Insurance and are underwritten by an A.M. Best "A" Rated Insurance Company.Accident Medical and AD&D benefits will not be provided through the association which provides those benefits in Connecticut, Indiana, N.Y., N.J., Vermont and Washington. If you live in one of those states The Total Care Plan or Value Care Plan provide great benefits and are cost effective programs even without the Accident Medical Benefits and AD&D. No plans are available in the following states: Florida, Oklahoma, Kansas, North Dakota, South Dakota and Montana.

Emergency Rescue
Reimbursement includes the cost of medically necessary or life threatening helicopter transportation from the scene of an accident to the nearest medical facility capable of treating the injuries or from one medical facility to another medical facility (the latter item needs to be approved by the medical officer in charge of this service).

This benefit provides instant and possibly life saving personal medical information by telephone, fax or computer whenever you need it.  Information on medical conditions, allergies, insurance, immunizations, medications, physicians and emergency contact will be available to healthcare providers within minutes of your call.  Medifile has a complete system of security controls to ensure that records are released only with the member's authorization.
